If there's any constant with Baldur's Gate 3, it's that there's always more to discover, and even repeated playthroughs can often leave certain secrets or details unrevealed. The vast array of choices and possibilities is a large part of what's made the game so attractive, and many interesting outcomes can act as exciting little surprises whenever they're first triggered. On the other hand, it's also possible to encounter scenarios that are ultimately more unpleasant than anything, a natural consequence of telling a story in a fantasy world filled with violence and treachery.
Where things can get truly strange in Baldur's Gate 3 is when the attention to detail of developer Larian Studios collides with the bugs that can be found throughout the game. Bugs are also an unsurprising element in a game with so many possibilities, and although they're not as plentiful now as they were at launch, it's still very possible to come across issues every now and then. One example of an easily-missed detail colliding with an unintended interaction lies in a particular Karlach encounter, and the results are surprisingly grisly.

As showcased in a Reddit post from D-snut-s, it's possible to end up in a confrontation with a headless version of Karlach, and it's not a kid-friendly, cartoony depiction of the concept. Instead, the beheaded tiefling sports a horrifically authentic severed neck, and it's shocking just how detailed the texture work is. Baldur's Gate 3 definitely earns its M rating at times, and although this particular example isn't likely to crop up in most campaigns, it certainly assists in proving that point.
It's entirely unnecessary for Karlach to sport such a detailed beheaded model, as the hostile conversation depicted in the post is supposed to play out with a normal Karlach model. A group pretending to be Paladins in Act 1 of Baldur's Gate 3 requests both Karlach's death and her head brought to them as proof, so anyone who follows this route is given the option to chop through her neck. The body, however, is simply meant to lay on the ground after the head is recovered, and a basic texture with just a few pixels would have essentially covered things.
It can initially seem like it may be necessary to kill Karlach to recruit Wyll, but both can ultimately join the party without issue.

The scene in which Karlach appears headless seems to be a mistaken trigger for her reaction to the destruction of the Grove in Act 1. If the party chooses to kill the inhabitants of the grove while Karlach is already in the party, the tiefling makes a quick departure from the party, but meeting her for the first time after committing this crime will result in a direct confrontation. It obviously isn't supposed to happen if Karlach is dead and beheaded, but it certainly makes for a memorable encounter.

It's unclear whether D-snut-s got the unusual Karlach scene by accident or by a direct attempt at manipulating the situation, as the easiest way to end up with a walking, talking, headless Karlach has traditionally been by killing her and reviving her. Pulling this off has been more convoluted and less consistent since Baldur's Gate 3 Patch 4 addressed the basic problem, however, and it still doesn't quite account for this particular scene. Killing Karlach requires meeting her, which itself is supposed to disallow this particular scene from occurring.

One screenshot of a conversation with a decapitated Karlach is probably more than enough for most Baldur's Gate 3 players, as the tiefling has a decidedly less discomforting appearance when she remains in possession of her head. At any rate, the disturbing precision of the depiction shows just how far the series has come from the simpler graphics of the first two games. Larian Studios might not have managed to make Baldur's Gate 3 a watertight experience in terms of bugs, but the company certainly ensured that even unintended moments can still have the capacity to impress.
