Starfield is a rich world that is filled with a few interesting and random encounters if players scour the Settled Systems for a long enough time. While some encounters are pretty commonplace — like the terrifying experience with the Aliens-esque xenomorph Interloper ship, others are much harder to find. In fact, these random encounters are so rare that it's more than possible not to stumble upon them after hundreds or even thousands of hours of gameplay.
One such user named BaaNaaNaa on Reddit found out after thousands of hours exploring the known galaxy in Starfield and completing the main quests and side missions, yet never stumbling upon the derelict abandoned ship the community has come to know as the "Pale Lady." While this normally wouldn't be anything surprising, as the Settled Systems are quite large and include unique POIs and random encounters many never find, it is a bit more surprising that this one encounter never occurred as the user has over 1,500 hours of gameplay.
Considering that they state they are "both saddened and relieved," proves how spooky this occurrence actually is, as random low-chance encounters like this are typically welcomed with open arms by Starfield players. This encounter is so rare, apparently, that even though they weren't seeking it out, most would assume that they would have stumbled upon it at least once throughout their time with the game.

The Pale Lady is a random Starfield encounter with a low chance of spawning. The quest will start in a random star system, where a Derelict UC Longsword spaceship will be floating out in space simply titled "Ship." Approaching the ship will initiate an Activity that instructs the player to "Board the derelict UC Longsword II," while a creepy, haunting laugh rings out. User Fernades on YouTube documents the entire encounter on their channel.
Upon boarding, the player will find the crew all deceased on the ship, with items overturned and strewn on the ground. A note titled "Insomnia" details one of the crew members' difficulty in sleeping due to the Pale Lady story. Eventually, a crew log can be found, which uncovers more details about the unfortunate circumstances on the ship. Two crew can be heard speaking as the Pale Lady's laugh rings out in the background. If the player stays on the ship long enough, they'll also hear the Pale Lady's laugh.

The Pale Lady is actually a monster and minor character who appears in the 14th story of Scary Stories 3: More Tales to Chill your Bones entitled, "The Dream." She is also the antagonist from the movie based on the books and is a huge, monstrous ghost who stalks her victims in those properties. Obviously, Bethesda's developers and storytellers are big horror fans, as there are other references to similar media packed within Starfield.
