"The Fall" is set up perfectly by A Gentleman in Moscow episode 5, making Alexander Rostov's life in the Metropol Hotel even more bearable for the former count. Certain members of A Gentleman in Moscow's cast continue their astounding transformations, whereas some remain more themselves than ever.

After a short sequence showing a young Sofia doing what her new father figure can't do and leaving the hotel to go to school, A Gentleman in Moscow episode 6, "The Fall," jumps forward another 11 years. In total, this means it's been a quarter of a century since Alexander last left the walls of the Metropol Hotel. Sofia is now becoming a young woman, much to Alexander's visible chagrin, and other characters have experienced similar changes in their lives.
According to Soviet History, 1947 marked the end of the Soviet Famine, although this isn't referenced in "The Fall." For Alexander, aside from being imprisoned in the hotel, the goings-on outside have almost no impact on his day-to-day life. While the time jumps have a notable impact on most of the characters in A Gentleman in Moscow, Alexander largely just gets older. However, the altered trajectories of his friends and loved ones do also affect him.

In the previous episode, Sofia referred to McGregor's character as, "Uncle Alexander." Count Rostov was only ever intended as a part-time caretaker for Nina's daughter, with the plan being that his friend would eventually return. While Alexander and Sofia are unaware of Nina's death, it was confirmed in A Gentleman in Moscow episode 5, "An Arrival." Now that years have passed, the bond between Alexander and Sofia has become that of a father and daughter. This is proven by their interactions, and the fact that Sofia even calls Alexander "Papa."

Following Sofia's terrifying fall, Alexander Rostov puts his own fate to one side and darts from the Metropol Hotel with his injured daughter in his arms. Although Nina's key means he has been able to head to the building's roof for some fresh air, this shocking moment marks the first time Alexander has left the hotel in 25 years. Although he doesn't react too noticeably, the new version of Moscow is likely very different from the one he last laid eyes on.
Thankfully, Alexander has made so much of an impact on the staff of the Metropol that they put their lives on the line and lie to the Red Army about what they saw.

Manager Halecki isn't in A Gentleman in Moscow episode 6. Instead, Leplevsky has been promoted to manager at some point during the 11-year time jump. Thankfully, Leplevsky has either failed to find a way to get rid of Sofia from the hotel or has given up or even come to begrudgingly accept her presence. His character has been teased as a threat from very early in the show's run. Beginning as a waiter in the hotel restaurant, Leplevsky has come a long way, and he has made it clear he dislikes Alexander at every opportunity.
The new manager of the Metropol continues his villainous ways in "The Fall" by reporting Alexander's unlawful departure from the hotel. While it could be argued that Leplevksy was simply performing his duties, he isn't coy about the fact that he seems to be enjoying the possibility of Alexander finally being out of his hair after a quarter of a century. Luckily, the staff and Anna rally around Alexander and manage to avoid his execution, but Leplevsky will surely not give up.

Sofia fell down several flights of stairs and sustained a horrifying head injury. As well as a fractured skull, she sustained a bleed on the brain and immediately lost consciousness as well as a lot of blood. Even in modern times, the injury could very well be fatal. In 1946, medical care was not anywhere near what it is today, so while Sofia did receive surgery, she was very lucky not to succumb to her injuries.
Alexander was told as far back as A Gentleman in Moscow episode 1, "A Master of Circumstance," that death would be the penalty if he were to ever leave the Metropol Hotel. Although he was acting in his capacity as a devastated father, Alexander still broke the law by fleeing the building. As such, he was risking his own life to save his daughter. Thankfully, his ability to return without detection was his saving grace.

By the letter of the law, Osip Glebnikov should have arrested Alexander Rostov when he saw him in the hospital. Osip has been very coy about how he sees Alexander, as it's unclear if the pair are friends, uneasy acquaintances, or even just prisoner and jailer. Osip has shown leniency toward Alexander before, but helping him flee the hospital and return to the hotel undetected goes a step beyond turning a blind eye.

It would be difficult to describe Osip as anything other than Alexander's ally at this point, as there have been several occasions where Osip has acted unpredictably, in ways that go against his duties and instead start to favor Alexander. Her downfall began in A Gentleman in Moscow episode 3, "The Last Rostov," when Stalin's failure to attend her movie screening at the hotel led to her reputation being tarnished. Since then, she has been supplanted by a new star. Although she has continued to act, her fame has all but vanished.
When trying to flirt her way to safety with the Red Army captain, he doesn't recognize Anna Urbanova's name, which is a clear dagger to her heart. For someone who was once one of the biggest celebrities in her country, it must be very upsetting to no longer have the same level of influence. On the other hand, Anna hasn't starred in a movie for well over a decade by the time of "The Fall," so it's also not that surprising.

Nachevko is a relatively minor character in A Gentleman in Moscow, but hearing that he has been removed as the Minister for Culture - "Permanently" - is a worrying statement. When tasked by Osip with seeking information to take down Nachevko in A Gentleman in Moscow episode 4, "Good Times," Alexander's discovery regarding Nachevko's sexuality could have had the minister killed, but McGregor's conscience prevents him from sharing the secret with Osip.
The ominous tone of the exchange between Anna and the Red Army captain implies that Alexei's secret has since been discovered, and he has been brutally punished for it. However, this isn't confirmed in "The Fall." Nachevko played a huge part in helping Anna Urbanova achieve career success during her younger years, so she is understandably upset at the possibility of his demise.

However, the narration from the older Sofia strongly implies it's all going to fall apart soon.
