Many of these are first confirmed to exist via datamining, then found organically in-game after some experimentation, be they legendary gear or hidden locations. Some of them, though, are much more bizarre. This is one of those latter examples, a hidden quest that rewards a completely useless item. It may be a simple reference to an infamous item from Diablo 2, or something much deeper.
Diablo 4 players have discovered how to spawn the Ring of Misfortune, according to a Reddit post by Sowhatsthecatch. Although datamining confirmed its existence only shortly after launch, this is the first time in the ensuing year that a player has claimed to have found the ring. This unique item plays on the supposed unluckiness of the number 13, boosting various player stats by that factor. It also includes a Lucky Hit bonus that causes the player to lose all of their Resource (akin to mana or MP), making it actively dangerous to use in battle.
The Ring of Misfortune can only be obtained when five players, each of a different base class, each throw a Lucky Coin into the Wishing Well in Scosglen. This results in a meteor shower, after which everyone involved can pick up the unique item. It's a fun little secret quest, but its true significance is still up for debate.

A Diablo 2 Reference, A Cow Level Connection, Or Something Else?

The most obvious theory about the Ring of Misfortune's involves a subtle reference to a rare item from Diablo 2. In this previous series installment, many a player spent countless hours farming in an attempt to find an exceptionally rare item called the Fortuitous Ring of Fortune.
This desirable accessory would boost the player's chance of finding magic items, buffing their further farming efforts by a factor of up to 40%. However, it was incredibly difficult to find, especially at the highest possible level, leading many players to dub it the "Fortuitous Ring of Misfortune" to honor their hundreds of failed attempts.
This theory is supported by the method by which players can obtain the Ring of Misfortune in Diablo 4. Reddit user Obliivescence once calculated that the average player would have to gamble away the equivalent of 10.4 billion gold in order to find a Fortuitous Ring of Misfortune. Considering that players are literally throwing their Lucky Coins away in order to obtain the Ring of Misfortune, this seems a likely explanation for the ring's existence.

A mere community meme during the Diablo 1 days, the cow level became a reality in Diablo 2, complete with a boss called the Cow King.
